Plague Inc: Evolved is a real-time strategy simulation game developed and published by the UK indie studio Ndemic Creations. It is a high-definition remake of the developer’s original 2012 mobile title Plague Inc., rebuilt in Unity with improved graphics, user interface, statistics, achievements, and Steam Workshop support. The game entered Steam Early Access on February 20, 2014, and launched in full on February 18, 2016, for PC.

Players start by infecting Patient Zero and must evolve a pathogen to spread across the world, and ultimately wipe out humanity. The simulation uses a detailed epidemic model with hundreds of variables covering transmission, symptoms, environmental resistance, and lethality. It features 10 different disease types —  Bacteria, Virus, Fungus, Parasite, Prion, Nano-Virus, Neurax Worm, and moree — each with distinct mechanics.

Infections earn DNA points, which are used for evolving traits across separate tabs for transmission, symptoms, and abilities. Country-specific responses, dynamic world events, and cure research add strategic depth. Additional modes include Versus and Cco-op multiplayer, a Scenario Creator, and full Steam Workshop integration for user-generated content.

Plague Inc: Evolved has accumulated about 60,500 reviews on Steam and holds a 94% positive rating. It has been praised for its strategic complexity and realistic disease modeling, and the franchise as a whole has reached more than 190 million players across platforms. Recent Steam concurrent player counts typically stayed below 1,000 before the latest update and sale, which saw it explode to over 8,000.

Plague Inc: Evolved received a major content update on April 7, 2026, with the Aliens & Anti-Vaxxers DLC, which added the new Xenolith plague type and six themed scenarios. A free Update 1.23 released alongside it introduced the official Leprosy scenario and balance changes.

At just $2.99, the game is currently at its lowest-ever price on Steam. This 80% discount deal is part of the ongoing Hidden Object Fest, which ends on April 23, 2026. The deal applies to the base game only, but all of its DLCs are also on sale right now.
